For Patient's Requesting a Single Clinical Note or Imaging:

Please visit your SOUC Patient Portal or call/visit your nearest Sport Ortho Location. They will handle everything for you with a smile.

For Entire Patient Record Requests:

1. Please click the link below to submit a medical records request; Do NOT submit your request in person.

2. You will be asked for a payment with a one-time charge of $20.

3. You should receive your records within 5 business days of completing your request.

1. For third-parties, such as attorney's and insurance companies, needing to request a copy of a patient's medical records please fax your request to 615-777-3517.
